Conceptual Framework

The Conceptual Framework demonstrates how the Center for Community Health links inputs from the community with enhanced community capacity for prevention and overall improved community and population health.  Key components of this framework are ongoing evaluation of the process and recognition of the context in which health disparities occur.  The Center for Community Health Conceptual Framework is modeled after the Centers for Disease Control and Prevention's Logic Model for the Prevention Research Center Program.
